The FUSE enigma: Wide-angle or wide-minded?

Although the event is rare, it remains an undeniable fact that approximately 1 in every 1500 colonoscopies is later followed by a postcolonoscopy cancer.1 The most immediate reaction is to assume it is a de novo cancer. This is the most persuasive and convenient—and defendable—argument for both ourselves as endoscopists and our patients. It is also indirectly supported by epidemiologic data, which show a surge of proximal cancer in the elderly with lower, if any, protection by screening colonoscopy.
