The dawn of a new era: performance measurement for the pathologic diagnosis of Barrett’s esophagus–associated dysplasia

Esophageal columnar metaplastic epithelium (ie, Barrett's esophagus [BE]) carries the risk of progressive transformation into low-grade dysplasia (LGD) and high-grade dysplasia (HGD) and eventually adenocarcinoma.1,2 Endoscopic inspection with biopsy remains the cornerstone of surveillance of BE patients for identifying dysplasia and early malignancy. As a result, the histopathologic diagnosis plays a pivotal role in clinical decision making and treatment of patients. However, the diagnosis of dysplasia, particularly LGD, in BE can be challenging and highly variable.
