In this issue of Brain, Alex Doets, Christine Verboon, Bianca van den Berg and colleagues report the results of an international study of Guillain-Barré syndrome, which shows a higher frequency of pure motor presentation in Bangladesh, and of axonal subtype on electrophysiology, than in Europe/Americas and the rest of Asia. They also report regional differences in the incidence of the Miller Fisher variant and in outcome. It is unclear to what extent these differences relate to different rates of exposure to Campylobacter jejuni, which has been blamed for seasonal fluctuations in axonal Guillain-Barré syndrome in China. The epidemiology of neuro-immunological disease will no doubt continue to evolve, as witnessed by outbreaks of Guillain-Barré syndrome in South America shortly after the arrival of Zika virus on the continent a few years ago.
