Differences in United States insurance payer policies and American Society for Radiation Oncology’s (ASTRO) model policy on stereotactic body radiation therapy (SBRT)

Insurance payers in the United States vary in the indications for which they consider SBRT "medically necessary". Since the last update to ASTRO's model SBRT policy, the proportion of payers considering SBRT "medically necessary" for most indications has increased. National payers and benefits managers consider SBRT "medically necessary" more often than regional payers. However, early stage lung cancer and spinal reirradiation are the only indications covered in ≥90% of policies, and advocacy is needed.
