Closer to the Source: Targeted-Release Corticosteroids for Immunoglobulin A Nephropathy

The efficacy and tolerability of systemic immunotherapy in immunoglobulin A nephropathy (IgAN) continues to be a source of intense discussion and investigation. Recent randomized trials have confirmed the substantial toxicity of systemic immunotherapy, but have also highlighted potential efficacy with respect to achieving proteinuria reduction.1,2 These results emphasize the need for less toxic treatment options that more directly target the immunopathogenic mechanisms responsible for IgAN. Can we achieve this panacea through local direct modulation of IgA production with minimal systemic drug exposure and toxicity? Results of a phase 2b trial of a drug engineered with this exact purpose have recently been reported.
