Male With Severe Headache

A 57-year-old man with cervical radiculopathy presented to the emergency department with a severe global headache that began immediately after a midline cervical (C5 to C6 level) epidural injection of bupivacaine, methylprednisolone, and radiographic contrast performed at an interventional pain clinic. The headache was unremitting and associated with repeated episodes of vomiting. The neurologic examination was entirely normal. Computed tomography (CT) of the brain was obtained (Figures 1 and 2).
