We thank Dr Steenholdt for his interest in our work, but it seems that he has misinterpreted the point of our study. We would like to stress that the main purpose of this observational retrospective study was to investigate the long-term outcomes of 2 different therapeutic strategies: proactive and reactive therapeutic drug monitoring (TDM). Although reactive TDM has been shown to be more cost-effective than empiric dose-escalation,1 for many patients reactive TDM is too late for dose optimization, because high titer antidrug antibodies and/or severe intestinal inflammation may have already developed.
