Abstract Cutaneous lei shmaniasis presents a very characteristic clinic of erythematous-yellowish papule with central crust, in exposed regions, that we health professionals can identify and treat without the need for a biopsy. Sometimes, it presents with an atypical clinic that can lead to a diagnostic error, with its perpetuation and discomfort in the patient. We present the case of a 53-year-old woman with an erythematous-exudative plaque throughout the extension of the left atrial pavilion of several months of evolution. It had been treated from the start with antiobiotherapy and corticosteroids, without achieving improvement and with a clear worsening, as it had spread throughout the ear. After the biopsy was performed, the diagnosis of erysipeloid cutaneous leishmaniasis was reached. Directed treatment with fluconazole and subsequent infiltrations of meglumine antimoniate were made, achieving its complete resolution.

Abstract One of the cornerstones in the management of coronavirus pandemic is the early identification of virus presence in patients. The most used test is the nasopharyngeal swab, used to amplify nucleic acids through polymerase chain reaction. Complications with this test have not been completely characterized. Until now, only one international report of cerebrospinal fluid leak has been reported. We present two cases of leak after nasopharyngeal swab test: the first case corresponded to an adult feminine gender patient with suspected idiopathic intracranial hypertension, whose gap was surgically repaired; the second case adult male patient with medical history of hydrocephalus and neonatal meningitis who was further studied for rhinoliquorrhea that showed a meningoencephalocele occupying the right frontal recess.

Abstract Introduction: Mucosal bridges (MB) correspond to a mucosa loop parallel to the free edge of the vocal fold, which is joined forward and backward. There is not a standardized procedure for its treatment and the literature in this regard is scarce and surgical management have not resulted in predictable outcomes. Aim: Description of clinical characteristics of patients with MB and the surgical techniques, in the Otorhinolaryngology Service at the Clinical Hospital Universidad de Chile (HCUCH), reflecting upon the challenge for its diagnosis and management. Material and Method: Retrospective and descriptive study, including patients with diagnosis of MB, between 2013 and 2019 in HCUCH. The preoperative evaluation includes anamnesis, physical examination, nasofibroscopy and/or telelaryngoscopy, along with the use of stroboscopy. Results: 7 cases were included, with an average age of 37.4 years; all women, with diagnosis made in the operating room, on a single vocal fold. It was associated with another minimal structural abnormality (MSA) in 100% of the cases, and with benign pathology in 75% of them. Dysphonia was the main symptom. The treatment was chosen individually, with resection in all cases, various laryngeal microsurgery techniques, infiltration of steroids and/or fat, in addition to speech therapy. Conclusion: MB should be suspected in cases of chronic dysphonia with an unfavorable response to speech therapy, in association with other MSAs, particularly in women, diagnosed intraoperatively with adequate equipment, including informed consent for eventual surgical management. It is a rare pathology, whose management has not been standardized therefore our experience is relevant.

Abstract Introduction: Simple chronic otitis media (COM) is a common pathology in our population, and it is currently unclear, which grafts technique and material gives the best results. Aim: To determine if there are differences in the anatomical and functional results, when using temporal muscle fascia (FMT) or tragus cartilage graft (CT) in patients with COM who underwent myringoplasty at the Barros Luco Trudeau Hospital. Material and Method: Retrospective analytical cohort study of data obtained from clinical records. Results: Of 227 medical records, 154 met inclusion criteria. FMT was used in 102 pa tients (66%) and CT in 52 patients (34%). With FMT, 38 had reperforation (37%) and 41 had functional success (40%). With CT 18 had reperforation (35%) and 22 had functional success (42%). 38 patients had a history of active smoking and 53% of them presented reperforation, while of those without a history of smoking only 31%, this difference being statistically significant (p < 0,05). Conclusion: No statistically significant differences were obtained when analyzing the anatomical and functional results comparing the use of FMT and CT graft, for the simple surgical treatment of COM with myringoplasty.

Abstract Introduction: Ménière's disease is characterized by recurrent episodes of vertigo, sensorineural hearing loss and aural fullness, which can be incapacitating and cause stress and anxiety. Aim: To assess the effect of transtympanic steroid injections on hearing and vertigo for patients suf fering from intractable Ménière's disease. Material and Method: Patients with intractable Ménière's disease were recruited and treated with three transtympanic steroid injections (dexamethasone 4 mg/ml). Audiometric and vestibular assessment, and two dizziness questionnaires (DHI, UCLA-DQ) were performed upon recruitment, three weeks, and ten weeks following the transtympanic treatment. A brief auto-perception survey was applied at 6 months follow-up. Results: Twelve patients completed the study. An increase in the mean auditory threshold was observed up to 11.25 dB, mainly between 125 and 3000 Hz at three weeks, and a reduction of these up to 7.5 dB at ten weeks with respect to the initial ones, mainly between 125 and 2000 Hz (not statistically significant). All patients presented vestibular dysfunction on caloric testing. A significant impro-vement in the UCLA-DQ survey score was observed, remaining throughout the follow-up. No statistically significant differences were found in the overall score or subcomponents of the DHI. Seven out of eight patients reported that they would undergo the injections again. Conclusion: Transtympanic corticosteroids can help to reduce disability and impact on the quality of life of patients with Ménière's disease that is difficult to manage in the medium and long term.

In Vivo. 2021 Jul-Aug;35(4):2439-2444. doi: 10.21873/invivo.12522.

ABSTRACT

BACKGROUND/AIM: Oncotype DX recurrence score (RS) for breast cancer is a useful tool for determining chemotherapy indication but it is expensive and time-consuming. We determined whether four immuno-histochemical markers, namely human epidermal growth factor 2 (HER2), estrogen receptor (ER), progesterone receptor (PgR), and Ki-67, are predictive of an RS ≥26 in Japanese patients.

PATIENTS AND METHODS: The study included 95 Japanese patients evaluated for RS. A predictive model was created using logistic regression analysis.

RESULTS: The discriminant function was calculated as follows: p=1/{1+exp [-(4.611+1.2342×HER2-0.0813×ER- 0.0489 ×PgR+0.0857×Ki67)]}. Using a probability of 0.5 as the cutoff, the accuracy, sensitivity, specificity, positive predictive and negative predictive values were 90.5%, 72.2%, 94.8%, 76.4% and 93.5%, respectively.

CONCLUSION: The model had a high negative predictive value in predicting RS ≥26 in Japanese patients, indicating that Oncotype DX testing may be omitted in patients with a negative result according to the predictive model.
