Preserve the Facial Nerve

Magnetic resonance imaging shows typical radiographic features of a vestibular schwannoma (Koos grade 4, tumor deforming the pons, and shifting of the fourth ventricle), including the lack of surrounding edema in the compressed pons and the extension into the internal acoustic meatus.1 Typically, a neuroradiological diagnosis of vestibular schwannoma is sufficient and no biopsy is required. Beause lack of hearing is Gardner-Robertson grade 5 in the right ear, preservation of the right V and VII nerve functions are most important.
