Three-Month Old with a Tongue Mass

A 3-month old healthy infant boy presented to the emergency department for evaluation of a swollen tongue (Figure). Soon after birth, his parents had noted a small mass on the undersurface of his tongue. His pediatrician recommended an outpatient ears, nose, and throat follow-up as there were no accompanying symptoms. Over the next 2 weeks, the swelling progressively worsened leading to feeding difficulties, choking episodes, and noisy breathing. He was noted to have upper respiratory symptoms including cough, rhinorrhea, and fever.
