Acute Kidney Injury in a Patient Following Kidney Transplantation

A 66-year-old man with kidney failure was maintained on hemodialysis therapy for 2 years before receiving a deceased donor transplant. He developed acute kidney disease (AKI) requiring dialysis during septic shock following an incarcerated hernia; his estimated glomerular filtration rate (eGFR) was normal before this episode and he did not recover eGFR. He never underwent a kidney biopsy, and his kidney failure was attributed to acute tubular necrosis. His medical history was also notable for hepatitis C virus (HCV) infection, hypertension, and a monoclonal gammopathy.
