Pulmonary Hypertension or Cardiorenal Syndrome: The Devil Is in the Details

I read with great interest the recent article by Dr Tang et al.1 In their meta-analysis of 16 studies, the authors concluded that pulmonary hypertension is associated with increased risk for adverse outcomes in kidney disease. Interestingly, the selection criteria called for studies defining pulmonary hypertension only as increased pulmonary artery systolic pressure. Group 2 pulmonary hypertension (ie, postcapillary pulmonary hypertension due to left cardiac dysfunction) has the highest prevalence among all groups, and 60% to 80% of patients with left cardiac dysfunction have some degree of pulmonary hypertension.
