The article by Kochhar et al1 emphasized the advances in imaging technologies that showed promise in the early detection of dysplasia and cancer. It is true that electronic chromoendoscopy such as narrow-band imaging, linked color imaging, and blue laser imaging has facilitated the evaluation of superficial gastrointestinal neoplasia. However, endoscopists with advanced skills are not always available and the accuracy of optical diagnosis in a nonacademic setting may be suboptimal.2,3 In addition, even expert eyes can hardly capture all the minute lesions with electronic spectral enhancement.
