Pseudopseudohypoparathyroidism: A Diagnostic Consideration in a Patient with Brachydactyly

A 17-year-old girl was evaluated in the emergency department for post-traumatic pain to her right hand. The patient's past medical history was unremarkable other than for mild global developmental delay. On physical examination, the patient looked well with bilateral symmetric shortening of the third, fourth, and fifth digits. Hand radiographs revealed symmetric short third, fourth, and fifth metacarpal bones and shortening of the distal phalanges bilaterally (Figure). Upon further questioning, the patients' father was also known to have short digits.
