We read with interest the findings by Le Berre et al1 reporting the first case combining a biologic agent and tofacitinib for the management of inflammatory bowel disease (IBD). The recent approval of the small molecule tofacitinib for the treatment of ulcerative colitis, and the multiple small molecules in the therapeutic pipeline, makes this combination an increasingly attractive approach as we try to close the therapeutic gap in IBD. Furthermore, their lack of immunogenicity offers an advantage over biologic agents, potentially allowing for sequential therapy.
