Patterns of Failure and Survival Outcomes After Total Lymphoid Irradiation and High-Dose Chemotherapy With Autologous Stem Cell Transplantation for Relapsed or Refractory Classical Hodgkin Lymphoma

The strategy of combined sequential salvage chemotherapy, total lymphoid irradiation, high-dose chemotherapy and transplant has been shown to result in good long-term outcomes in patients with refractory or recurrent Hodgkin lymphoma. We used the posttreatment scans to precisely study the patterns of failure and found that most patients failed less than a year after the transplant within the irradiated volume, reflecting treatment-resistant biology. Efforts should continue to identify more effective systemic treatments.
