Tumor-infiltrating T-cell Receptor-beta Repertoires are Linked to the Risk of Late Chemoradiation-induced Temporal Lobe Necrosis in Locally Advanced Nasopharyngeal Carcinoma

A brisk neuroinflammatory response is a hallmark of radiation brain injury. There has been limited assessment of the impact of immunological effects on radiation toxicity in the central nervous system. The roles of blood circulating neutrophils and tumor-infiltrating lymphocytes in chemoradiation-induced temporal lobe necrosis (TLN) in patients with nasopharyngeal carcinoma (NPC) have not been established. We used pretreatment tumor tissues and blood samples from nonmetastatic T4 NPC patients with intracranial invasion and showed that some specific intratumoral T-cell receptor-beta repertoire subtypes and increased baseline or post-chemoradiotherapy neutrophil-to-lymphocyte ratios were linked to TLN susceptibility. These data underscore the significance of an immune impact on the chronic pathogenesis of chemoradiation-induced late TLN in NPC patients and indicate the potential for immune-based therapies.
