In Reply to ‘Core Assessment of Predonation Kidney Function: Clarification of the 2017 KDIGO Living Donor Guideline’

We thank the KDIGO guideline committee for their interest1 in our Core Curriculum.2 The minimum acceptable glomerular filtration rate for prospective living kidney donors that we proposed (80 mL/min) was more stringent than the threshold suggested by the KDIGO group3 (60-89 mL/min). The objective of our review was to provide a comprehensive assessment of current practice in living kidney donation aimed at the needs of clinically active nephrologists. In the absence of sufficient gradable evidence on which to base any firm recommendations, we suggested a more conservative minimum acceptable glomerular filtration rate than recommended by the KDIGO group that was consistent with a majority of transplantation center practices surveyed in 2007,4 as well as our own.
