Altered Prostaglandin Signaling as a Cause of Thiazide-Induced Hyponatremia

Thiazide diuretics are widely used in the management of hypertension. Although generally well tolerated, use of these drugs can cause hyponatremia, with an incidence ranging from 13% to as high as 30%.1,2 Risk factors include advanced age, female sex, low body mass index, and initiation of treatment with the drug during the summer months.3 The natriuretic properties of the drug, leading to baroreceptor-mediated secretion of arginine vasopressin (AVP), likely explain the pathogenesis of this disorder in a broad category of patients who exhibit clinical and laboratory evidence of decreased extracellular fluid volume.
