Tiny babies can be sugary sweet—how hard should we work to control this?

Hyperglycemia is one of the more common biochemical abnormalities encountered after preterm birth. Importantly, this abnormality is associated with many neonatal morbidities and an increased risk of mortality. Thus, it stands to reason that preventing hyperglycemia would result in improved outcomes in at risk neonates. In this volume of The Journal, Tottman et al present 7-year outcomes of hyperglycemic very low birth weight infants that were randomized to standard (<180 mg/dL) or tight (<155 mg/dL without insulin or 72-108 mg/dL on insulin) glycemic control.
