Adult-Onset Proximal Tubule Dysfunction

A 50-year-old African American woman presented to our clinic with an elevated serum creatinine concentration of 2.03mg/dL (estimated glomerular filtration rate of 32mL/min/1.73m2 by the 4-variable MDRD [Modification of Diet in Renal Disease] Study equation). A year ago, her serum creatinine concentration was 0.99mg/dL (estimated glomerular filtration rate > 60mL/min/1.73m2). Medical history was significant for severe obesity (body mass index of 34kg/m2), for which she was successfully undergoing a physician-supervised weight-loss program of 13 months' duration.
