Pilot Pharmacokinetic Study of High-Dose Daptomycin in Hemodialysis Patients With Infected Medical Devices

Long-term venous catheter use in HD is increasingly common and may sometimes be necessary. With its activity toward biofilm-embedded bacteria,1 rapid concentration-dependent bactericidal activity, and excellent tolerance, daptomycin is an optimal alternative to vancomycin for infected catheters. Although daptomycin is recommended at high doses (10-12mg/kg/d) in patients without kidney failure,2 in HD patients, it has only been studied at low dosages (4-6mg/kg at the end of each HD session).3-5 We conducted an open prospective pilot study to assess the pharmacokinetics of high-dose daptomycin (10mg/kg) in maintenance HD patients.
